Student Smart Card
Student smart card is an identity card
for Limkokwing University students, with an embedded microchip that can
be loaded with data, used for access computer labs, e-library, goods,
services purchased and etc. within the campus.
Application
- All
full-time staff and students of LUCT, LELC and IH who is provided a
smartcard is automatically entitled to the use of the smartcard for
purchases unless such right is revoked by MDI.
- The
Cardholder’s usage of the smartcard for top-ups and purchases
constitutes the cardholder’s agreement to the Terms and Conditions of
this Card.
- The cardholder is liable to inform MDI of any changes in the status as a full-time staff of students of LUCT, LELC and IH.
Replacement cards
- If
any card is faulty or damaged, the cardholder must fill a complaint
form at any Smartcard Counter to obtain a replacement smartcard. The
replacement Card will be sent within 3 days from the date on the
complaint form.
- A replacement fee will be charged.
- If
a smartcard is replaced due to theft or loss, it is de-activated and
cannot be re-activated should the cardholder find it at a later date.
Refund of Deposit and unutilized balance
- The
cardholder must fill a Card Return/Complaint Form to claim for a refund
of the deposit or any unutilized balance on the Card.
- If
MDI terminates this agreement, the cardholder may request a refund on
the unutilized balance within 30 days of MDI termination.
- Any
unutilized balance on the smartcard shall be refunded to the cardholder
within 30 days upon surrender of the smartcard less any administrative
charges incurred by MDI.
- Any unutilized balance on a
faulty card shall only be refunded to the Cardholder within 30 days
upon the surrender of the smartcard and provided that the last card
transaction occurred within 120 days prior to the surrender of the
smartcard less any administrative charges incurred by MDI.
